The Community Connections Support Program (CCSP) is a federally funded initiative that helps community members who are experiencing — or are at risk of — social isolation to reconnect with their community.
Our caseworkers provide one-on-one support, social connection activities, information and referrals to mainstream services, and culturally responsive group programs.
